Job Type: Full time, Hourly

Wage: $18.34-$23.40/hr.

Location: Canon City, CO.

Starpoint is seeking a detail-oriented individual with excellent communication skills to coordinate a caseload of Host Home Providers (HHP’s). Our HHP's provide support to persons with developmental and/or intellectual disabilities within their home, and to assist them to live the most fulfilling life possible in a family like environment. 


Position Duties and Responsibilities

  • Oversee the coordination and management of day-to-day caseload of Host Home Providers. 
  • Recruit, train, and monitor Host Home Providers and ensure contract compliance.
  • Hire, train, and monitor Respite Providers to ensure they are following all agency guidelines.
  • Programmatic coordination and monitoring in accordance with individual’s service plan.
  • Oversee all aspects of ensuring person centered supports for all adults enrolled in that program and that they experience a high quality of life based on their abilities and wants.
  • Create detailed individualized plans, tracking, and monitoring of the person’s progress.
  • Ensure agency and state licensing standards are adhered to in all Residential settings.




Requirements

QUALIFICATIONS:  

Possess a B.A. or B.S in related Human Service field or have five years of related experience working in the Developmental Disabilities field.

Must have a valid Colorado driver’s license and a certificate of personal auto insurance on file.

Must clear back-ground check, DORA check and 3 valid reference checks

Must be able to work scheduled hours and additional hours if requested by supervisor 

Possess Qualified Medication Administration Person (QMAP) certification, CPR, First Aid, and Safety Care training. 

Must have own telephone.  

Own and utilize reliable transportation for all aspects of employment. 

Willingness to travel to Colorado Springs, Pueblo, and throughout Fremont and Chaffee Counties.


Job Type: Full-time in person.  This position is not remote.
